Community Engagement IMPACT Practice Model | The Colorado Health Foundation At the foundation, community engagement is both a process and an outcome. the community engagement impact practice model provides the guiding principles that represent the actions and behaviors that program staff should embody. Hcpf engaged the colorado health institute (chi) to explore best practices in hospital engagement with communities as described in hb23 1243. hcpf and chi convened a stakeholder meeting on november 28, 2023, to solicit feedback on best practices and reducing redundancies.

Community Engagement IMPACT Practice Model | The Colorado Health Foundation There is a particular need in mental health, wherein growing rates of mental disorders and related deaths, especially among young people and historically underserved communities, call for new community engaged approaches, including prioriti zation of efforts with community leaders and organizations to assess and ac knowledge community strengths. These principles are outlined in our community engagement impact practice model, which was created to establish a “core set of ‘key functions’ that are critical to cultivating positive community change.”. Mdph is working to create a connected community of care in the region. the goal is to build connections and strengthen coordination among physical, mental, and behavioral health, human service, and community based service providers to offer more holistic care for individuals and families. The psd community engagement work group was part of a cdphe wide pilot charged with informing how all state agencies can implement practices to create more comprehensive and consistent community engagement across colorado.

As work began on the project, the organizing committee realized the need for a conceptual model illustrating the dynamic relationship between community engagement and improved health and health care outcomes. Below are several key best practices the department regularly considers or applies when planning and promoting public participation opportunities. At the colorado health foundation, our mission is that together, we will make colorado the healthiest state in the nation. we use the word “together” intentionally, because we truly believe that the only way for communities to reach their highest and healthiest potential is through collaboration. Equity focused foundations have generally sought to change institutions, systems, and structures through advocacy, policy change, and community organizing — either on their own or by supporting activist organizations.
