Evolving Our Practice Of Community Engagement | The Colorado Health Foundation Health and racial equity exists at the core of our community engagement work. we’re learning from community about the inequities they are experiencing, and use this learning to inform our strategic decisions on advancing health equity and addressing systemic racism. Below are several key best practices the department regularly considers or applies when planning and promoting public participation opportunities.

Community Engagement IMPACT Practice Model | The Colorado Health Foundation The psd community engagement work group was part of a cdphe wide pilot charged with informing how all state agencies can implement practices to create more comprehensive and consistent community engagement across colorado. From colorado’s western slope to the eastern plains and everywhere in between, we work in partnership with communities across our state to support and fund solutions to health challenges. we constantly listen to and learn from communities because they know best the need of their people. The colorado health foundation’s heal evolution learning to listen to community members was a critical step on the road to food security and good health for all coloradans. At the colorado health foundation, our mission is that together, we will make colorado the healthiest state in the nation. we use the word “together” intentionally, because we truly believe that the only way for communities to reach their highest and healthiest potential is through collaboration.

Community Engagement | The Colorado Health Foundation The colorado health foundation’s heal evolution learning to listen to community members was a critical step on the road to food security and good health for all coloradans. At the colorado health foundation, our mission is that together, we will make colorado the healthiest state in the nation. we use the word “together” intentionally, because we truly believe that the only way for communities to reach their highest and healthiest potential is through collaboration. Hcpf engaged the colorado health institute (chi) to explore best practices in hospital engagement with communities as described in hb23 1243. hcpf and chi convened a stakeholder meeting on november 28, 2023, to solicit feedback on best practices and reducing redundancies. Community engagement is an ongoing, evolving process of multidirectional communication with and for people to solve the problems and address the concerns that matter to them. the process should be durable, long lasting, and equitable to all who participate. We believe health is a basic human right, and that health status shouldn’t be dependent on where you live, how much money you make or the color of your skin. we recently redefined our organizational vision and strategic direction to focus on achieving health equity in all communities across colorado. This report encompasses a relentless pursuit of health and health equity, and it takes engagement on the ground to understand what is happening in and impacting communities across our state.
